Guidelines for Sending Shoes to SuperSoles

Before sending your shoes or filling out the google form to SuperSoles, please read the guildelines carefully so you can be assured you follow the process accordingly.
1) First, fill out the google form posted above so you are fully aware of the policies and accommodations you must comply with before sending your shoes. 
2) After completing the entire google form, place the number of shoes you are sending in a sealable box so all items will be sent safely. Aside from the shoes, ALSO place a piece of paper inside the box with your contact information. This includes your email, full name, and the number of shoes you are sending. Without your contact information sent with the box, we cannot be confident which pair of shoes we recieved from each person.
3) Thirdly, mail the sealed box with your pair of shoes AND contact information at your local post office. The SuperSoles mailing adress will be posted underneath this guideline discription.
4) Once the shoes have been delivered, we will immediately send you an email that your shoes have been sent accordingly and safely. If you filled out the community service hour form, your email will also include a signed certificate with the amount of hours you recieve.
5) If your school requires a specific form to be signed, please send the paper along with the box you are mailing to the organization. The certificate and the coordinating community service hour form will be sent together in the same envelope. If any other confrontations or confusions occur during the maling process, please email the organization with the posted contact information posted below.
